DAY CARE INSPECTIONS: ImagiNation Learning Center Cited for Safety Violations

daycareinspectionslogoThe Virginia Department of Social Services inspected ImagiNation Learning Center, located at 14421 Newbern Loop in Gainesville, Sept. 26 and cited the facility for safety and administrative violations.

Virginia Department of Social Services Comments:

An unannounced monitoring inspection was conducted with the director. Twenty children were present with five staff providing supervision. For lunch, the children were provided hamburgers, corn/peas, mixed fruit and milk. There were no medication on site for any of the children nor are there any children with allergies enrolled at this time. Documentation of injuries/accidents were reviewed. Four staff and children's files were reviewed today.

Violations

Standard #: 22VAC40-185-130-A

Description: Based on a review of four children's files, it was determined that the center did not have an immunization record on file for one child before the child was allowed to attend. Evidence: Child A enrolled on 9/6/16 and to date there was not immunization record on file.

Action to be Taken: We will make sure to obtain immunization records prior to their first day. We will get the immunization record for child A.

Standard #: 22VAC40-185-160-C

Description: Based on a review of four staff files, it was determined that two staff did not have a tuberculosis test/screenings/x-rays every two years. Evidence: Staff A's tuberculosis test was dated 7/31/14 and Staff B's tuberculosis test was dated 7/27/14.

Action to be Taken: We will review all staff files and make sure they have up to date tuberculosis tests/screenings/x-rays.

Standard #: 22VAC40-185-280-A

Description: Based on observation, it was determined that hazardous substances were not kept locked. Evidence: In the kitchen above the sink in an unlocked cabinet were 4 containers of Stermaine sanitizing tablets, stainless steel spay and multi purpose cleaner. In the Monkeys (two years) and Zebras (three years) classrooms were spray bottles of Oxivir disinfectant above the sink in unlocked cabinets.

Action to be Taken: All were locked during the inspection and we will talk with and retrain staff about the need to keep hazardous substances locked at all times.

Standard #: 22VAC40-185-330-C

Description: Based on observation, it was determined that ground support was not covered and presented a tripping hazard on the playground. Evidence: On the toddler/two year old playground the ground covered was exposed near the picnic table and presented a tripping hazard.

Action to be Taken: We will make sure to rake the shredded rubber to cover the ground covering.

Standard #: 22VAC40-185-440-J

Description: Based on observation, it was determined that there wasn't 30 inches of space between service sides of occupied cribs where the space is the walkway for staff to gain access to any occupied crib. Evidence: There was 12 inches of space between two occupied cribs.

Action to be Taken: We corrected the spacing during the inspection.

Standard #: 22VAC40-185-550-D

Description: Based on a review of the escape drill log and an interview with administration, it was determined that escape drills were not conducted monthly. Evidence: There was no monthly escape drill for July 2016.

Action to be Taken: We conducted two drills in August and we will remember to do them monthly.

The Virginia Department of Social Services inspects all licensed child-welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency: “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
