Day Care Inspections: Gainesville Children's Center

The Virginia Department of Social Services inspected the licensed child day care center Gainesville Children's Center, located at clear image used for formatting purposes only

8171 Linton Hall Road Feb. 19, 2013. The agency cited the facility with nine violations.

Virginia Department of Social Services Comments:

An unannounced monitoring inspection was conducted beginning at 10:40 A.M. and ending at 3:35 P.M. Violations were reviewed with administrative staff and will be emailed to center for completion of corrective action. Structured activities, centers, and lunch were observed. Two children's files and two staff files were reviewed. Medication was reviewed. Health and fire inspections are current. Not all standards were reviewed.

Violations

Standard #: 22VAC40-185-60-A

Description: Based on a review of two children's files, not all files contain the required information. Evidence: The files for children A and B do not have a signed statement from the parents authorizing emergency medical treatment. There is no such statement on any of the forms that the parents sign upon enrollment. The file for child A does not have a statement signed by the parent agreeing to notify the center within 24 hours of the child or a household member developing a contagious disease.

Action to be Taken: Contagious disease form and emergency forms will be given to each parent for every child giving us consent to treat, not only transport. Forms to be returned by 3/15/13.

Standard #: 22VAC40-185-70-A

Description: Based on a review of two staff files and staff interviews, not all files are complete. Evidence: The files for staff A and B, hired 2/18/13, did not have documentation of the required two reference checks conducted prior to hire date. Orientation documentation for staff A indicates that orientation was given by staff C. Interviews confirmed this. Orientation training is to be conducted by someone meeting program director qualifications. Staff C does not meet these qualifications. There is no documentation that staff B has received orientation, required by the end of the first day. There is no documentation of staff qualifications for staff B, a program leader. The documentation of qualifications for staff A were incomplete - there was no documentation of either a college diploma or transcript and only documentation of three of the required six months of experience.

Action to be Taken: As of 3/12 the last 6 reference checks are complete. Orientations are complete.

Standard #: 22VAC40-185-280-B

Description: Based on observation during a tour of the facility, not all hazardous materials are locked when not in use. Evidence: Expo white board cleaner, which is flammable, was on the top shelf under the children's calendar, unlocked, in the Junior Kindergarten room. Cleaning supplies in the upper right cabinet above the single sink in the toddler room were not locked. Hand sanitizer, also flammable, was in the diaper changing area in the toddler room, unlocked. Staff was not diapering children at the time.

Action to be Taken: All hazardous substances were locked during the inspection. Staff will monitor to ensure that all hazardous substances are locked when not in use.

Standard #: 22VAC40-185-510-C

Description: Based on a review of medications, expired medication is not returned to parents or disposed of. Evidence: EpiPen,Jr. for children C and D expired 1/13 and was still in the medication box at the center at the time of the inspection.

Action to be Taken: All expired medications are out of the center.

Standard #:22VAC40-185-510-J

Description: Based on observation during a review of medication, not all medication is locked, as required. Evidence: The center has two tackle box type containers holding medications, which are kept in a cabinet behind the front counter. The cabinet was not locked and only one of the two boxes of medication was locked.

Action to be Taken: The second box of medication was locked during the inspection. Staff will monitor to ensure all medication is locked at all times unless it is being administered to a child.

Standard #: 22VAC40-185-550-C

Description: Based on observation during a tour of the facility and staff interviews, emergency procedures and maps posted in classrooms. are incomplete. Evidence: The procedures posted in all rooms state the lead teacher is to designate who is to perform each duty, but there is no written designation as to who is actually assigned each responsibility. The posted maps for all rooms show all exits in the building. There is a horizontal line that extends from the toddler room exit to the kitchen exit, across the hall. There is a perpendicular line going from the horizontal line to the front exit. The legend on the map indicates this line is for evacuation and shelter-in-place. Staff interviews indicate classes evacuate through individual exits in each classroom and only the toddler class exits through that room. The posted map in the Junior Kindergarten room also shows three red lines, one going from the classroom to each of the three exits in the room. The legend indicates red is for evacuation, but does not indicate which is primary or secondary, as required.

Action to be Taken: Emergency procedures were re-written and posted. Emergency evacuation plan re-drawn and posted.

Standard #: 22VAC40-185-560-A

Description: Based on a review of posted daily schedules, there is not at least 1 1/2 hours and no more than 3 hours between snacks/meals, as required. Evidence: The posted schedule in the All Day Preschool room lists breakfast ending at 8:30 A.M. and A.M. snack starting at 9:45 A.M. The center opens at 6:30 A.M. Morning snack for the Junior Kindergarten class is listed at 10:00 A.M. with no breakfast. The posted schedule in the toddler room lists breakfast as ending at 8:30 A.M. and snack starting at 9:30 A.M.

Action to be Taken: Snack schedules were adjusted.

Standard #: 22VAC40-185-560-F

Description: Based on a review of the posted menu for the month of February, not all breakfasts and snacks meet the most recent USDA requirements. Evidence: 16 of the 20 morning snacks only have one of the required two components. 15 of the 20 afternoon snacks only have one of the required two components. 14 of 20 breakfasts only have one of the required three components. six of 20 breakfasts only have two of the required three components. None of the breakfasts list the required milk as a component.

Action to be Taken: Fresh fruit and milk is always a component at breakfast. Water and fresh fruit served with every snack.

Standard #: 22VAC40-185-560-G

Description: Based on a review of lunch boxes, not all lunch boxes are dated or labeled to identify the owner. Evidence: Two of five lunch boxes reviewed in the Junior Kindergarten room were not labeled to identify the owner. Two of five lunch boxes reviewed in the Morning Preschool room were not labeled to identify the owner. None were dated.

Action to be Taken: Letter sent to parents about food labeling, parents are complying.

The Virginia Department of Social Services inspects all licensed child welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency, “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
