DAY CARE INSPECTIONS: Montessori School of Gainesville Cited for Teacher Training, Record-Keeping Violations

The Virginia Department of Social Services inspected the Montessori School of Gainesville July 28. The agency noted violations in record-keeping, teacher training and cleanliness.

The day care center, located at 14421 Newbern Loop, was previously cited Nov. 14, 2013 for nine violations.

Virginia Department of Social Services Comments:

An unannounced monitoring inspection was conducted today with the owner. Sixteen children were present with five staff providing direct supervision. No children had medications on site today. Four children's files and three staff files were reviewed today. Staff interviews were conducted in each classroom during the initial walkthrough Fire inspection: 5/29/14 Health inspection: 7/14/14 Arrival time: 1:50 pm Departure time:5:10 pm

Violations

Standard #:  22VAC40-185-210-A-4

Description: Based on an inspection of the classrooms, it was determined that two staff were functioning as lead teachers but did not have the required six months of programmatic experience and/or the 24 hours of lead teacher training. Evidence: Staff A ( hired on 4/30/14) was functioning as a lead teacher in the 2/3 year old room; however she only has 3 months of programmatic experience. Staff B( hired on 5/30/14) is functioning as a lead teacher in the infant classroom but does not have the 24 hours of lead teacher training.

Action to be Taken: We will make sure Staff A does not function as a lead teacher and we will give staff B the training by 9/1/14.

Standard #:  22VAC40-185-420-E-3

Description: Based on an interview with Administration, it was determined that the parents of the preschoolers and infants are not provided at least semiannually in writing information on their child's development, behavior, adjustments and need.

Action to be Taken:  We will provide this information on the children.

Standard #:  22VAC40-185-500-B

Description: Based on observation and an interview with staff, it was determined that the diapering surface in the two/three year old classroom is not being properly sanitized. Evidence: The commercial chemical disinfectant the classroom was using did not indicate that it was 'hospital grade' disinfectant nor was there an EPA registration number on the bottle. Staff reported that they would spray the surface and then immediately wipe off the surface and not allow the surface to air dry for two minutes as required.

Action to be Taken: We will use the bleach water solution and we will remind staff to allow the surface to air dry for two minutes and then they can wipe it dry.

Standard #:  22VAC40-185-550-M

Description: Based on a review of nine injury/accident reports it was determined that the center failed to document all of the required information on the injury/accident forms. Evidence: On five of the nine forms the center failed to document the time of day parents were notified of injuries to their child.

Action to be Taken: We will make sure to document this information.

Standard #:  22VAC40-185-560-F

Description: Based on a review of the menu and staff interviews, it was determined that the center is not meeting the current USDA requirements. Evidence: For lunch a 16 and 18 month old had cheese sandwhiches and milk for lunch, there was no fruit/vegetable. For lunch the children in primary classroom ( 3.5 to 10 year old children) were served pasta in a tomato sauce, bananas and milk and there was no protein served

Action to be Taken: We will make sure to meet the USDA requirements.

The Virginia Department of Social Services inspects all licensed child-welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency: “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
