Day Care Inspections: SAC Programs Cited for Improper Files

The Virginia Department of Social Services routinely conducts compliance inspections of School-Age Care programs in Prince William County. SAC programs at Alvey Elementary, Glenkirk Elementary and Victory Elementary were all cited for failure to maintain required records in recent inspections.

Alvey Elementary SAC

Alvey Elementary SAC, located at 5300 Waverly Farm Road in Haymarket, received a monitoring inspection May 3 and was cited by the agency for an incomplete staff file.

Virginia Department of Social Services Comments:

A monitoring inspection was conducted today with the director, who was present with one additional staff and 11 children at the end of the inspection. The children were engaged in free play activities during most of the inspection. Several children had medications on site. A sample of children and staff files were reviewed today. Violations

Standard #: 22VAC15-51-40-C-2

Description: Based on a review of four staff files and an interview with the director, it was determined that one staff (A) did not have their central registry finding on file within 30 days of employment. Evidence: Staff A was hired on 8/15/12 and to date there was no documentation of a central registry finding for this staff.

Action to be Taken: I will check with corporate office to obtain the background check. If we do not have it this staff will not be allowed to return to the center until we have obtained the satisfactory check.

Standard #: 22VAC15-51-60-C-2

Description: Based on a interview with the director and a review of the staff files, it was determined that staff A was allowed to return to the center without having a central registry finding on file. This background check is required within 30 days of employment.

Action to be Taken: We will either find or resubmit this background check. Staff A won't be allowed to return to the center until we have obtained the satisfactory check.

Glenkirk Elementary SAC Program

Glenkirk Elementary SAC Program, located at 8584 Sedge Wren Drive in Gainesville, was cited April 2 for not having a staff member's files on the premises.

Virginia Department of Social Services Comments:

29 children were in care with four staff, The age range of the children was from five to 12 years old. Children were eating snack and small groups were going to computer lab. One staff was current in MAT The medications were complete. Sample of children's files were compete. Inspection time 4:50-5:50

Violations

Standard #: 22VAC40-185-70-A

Description: Based on review of staff records, the center did not have a file on all staff members. Evidence Staff A didn't have a file at center.

Action to be Taken: Staff A will carry their file with them.

Victory Elementary SAC Program

During a monitoring inspection, the Virginia Department of Social Services cited Victory Elementary SAC Program, located at 12001 Tygart Lake Drive in Bristow, April 26 for incomplete children's files.

Virginia Department of Social Services Comments:

A monitoring inspection was conducted today with the director who was present with two additional staff and 26 children at the end of the inspection. Two children had medications on site. The children were actively engaged in their center time at the beginning of the inspection. The risk assessed the violations were reviewed with the Director. Arrival time: 7 a.m., departure time: 8:05 a.m.

Standard #: 22VAC40-185-130-A

Description: Based on a review of six children's files and an interview with the director, it was determined that three children did not have immunizations on file, which is required before a child can attend. Evidence: Child A, B and C all enrolled on 9/4/12 and to date there were no immunizations on file.

Action to be Taken: We will get [it] from the school.

Standard #: 22VAC40-185-140-A

Description: Based on a review of six children's files and an interview with the director, it was determined that three children did not have physicals on file. Evidence: Child A, B and C all enrolled on 9/4/12 and to date do not have physicals on file within one month of enrollment as required by this standard.

Action to be Taken: I will obtain a copy from the school.

The Virginia Department of Social Services inspects all licensed child welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility, and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency, “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
