DAY CARE INSPECTIONS: Rainbow Station At Haymarket Cited for Safety Violation

daycareinspectionslogoThe Virginia Department of Social Services inspected Rainbow Station At Haymarket June 15 and cited the facility for safety and record-keeping violations.

The day care center, located at 6800 Piedmont Center Plaza in Gainesville, was previously cited Aug. 26, 2013, on a complaint for child mishandling.

Virginia Department of Social Services Comments:

114 Children were present during the inspection. The age range of the children was three months to five years. Children were observed during free play, outside time, lunch and group time. Ten staff records and children's records were reviewed. Inspection time 9:50 a.m.-1:15 p.m.

Violations

Standard #: 22VAC40-185-160-A

Description: Based on observation not all the staff records had a tuberculosis screening completed with in 21 days after employment. Evidence: Staff A's with a start date of March 1, 2015 record was missing the documentation of the tuberculosis screening.

Action to be Taken: Center will have staff complete the TB testing with in one next week.

Standard #: 22VAC40-185-280-B

Description: Based on observation not all hazardous substances were kept in a locked place. Evidence: There was antifreeze stored in the shed on the large playground. There are warnings on the antifreeze to keep out of the reach of children. The doors on shed were open and assessable to children.

Action to be Taken: A lock for the door will be purchased and the anti-freeze was removed.

Standard #: 22VAC40-185-510-C

Description: Based on observation not all the medications had a written authorization. Evidence: Child A's Epi-pen did not have a long term authorization for the administration of the medication signed by the physician.

Action to be Taken: Center will have the parent sign a 10 day authorization form for the Epi-pen. The parent will be provided the medical form to be signed by their physician.

Standard #: 22VAC40-185-550-D

Description: Based on observation evacuation drills were not completed on a monthly basis. Evidence: An evacuation drill was not completed during the month of May 2016.

Action to be Taken: Center will complete two evacuation drills during the month of June 2016.

The Virginia Department of Social Services inspects all licensed child-welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency: “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
