DAY CARE INSPECTIONS: Chesterbrook Academy hit with 9 Compliance Violations

daycareinspectionslogoThe Virginia Department of Social Services inspected Chesterbrook Academy July 6 and cited the facility for nine filing and safety violations.

The day care center, located at 7541 Virginia Oaks Drive in Gainesville, was previously cited Jan. 15, 2014.

Virginia Department of Social Services Comments:

An unannounced renewal inspection was conducted beginning at 9:30 a.m. and ended at 1:30 p.m.. There were 56 children present, ages 4 months through 6 years. All classrooms were in appropriate ratio. Activities observed included building, art, outdoor play, including sprinkler play, morning snack, card games. A newly enrolled child was very engaged in a water activity. Staff allowed her to adjust to her new classroom at her own pace. Reviewed medications, accident reports, children and staff files. Emergency escape drills and shelter in place drills are up to date as are health and fire inspection.

Violations

Standard #: 22VAC40-185-60-A

Description: Based on a review of the files for eight children's files, it was determined that two children's files were not complete. Evidence: The files for Child A and Child B, who are siblings were both missing emergency contacts, father's work place and phone and emergency medical authorization.

Action to be Taken: The director and the assistant will review all children's files to assure that they are complete and will review new admission paperwork at the time that the child starts.


Standard #: 22VAC40-185-70-A

Description: Based on a review of five staff files, it was determined that three staff were not complete. The staff files for Staff #1, 2 and 3 either had no references documented or had only one reference which was not dated.

Action to be Taken: The director will assure that the references are in the files.


Standard #: 22VAC40-185-270-A

Description: Based on observation, it was determined that there are areas that are not maintained in a safe manner. Evidence: The outside classroom doors to the Pre K I and Threes Classrooms have chipping paint.

Action to be Taken: The director will notify their maintenance department and the doors will be painted within a month.


Standard #: 22VAC40-185-280-B

Description: Based on observation, there were hazardous substances in an unlocked upper cabinet in the PreK I Classroom. Evidence: There was an unlocked upper cabinet that had 2 cans of shaving cream and a box of Alka Selzer in it.

Action to be Taken: The cabinet was locked during the inspection. Staff will be reminded that it is their responsibility to make sure that their cabinets are locked.


Standard #: 22VAC40-185-320-B

Description: Based on observation, it was determined that the water temperature in one classroom was higher than the maximum temperature allowed by standards. Evidence: The water temperature in the PreK Ii classroom was measured at at 125 degrees. The maximum allowable is 120 degrees.

Action to be Taken: The director will notify her maintenance department and the water heater temperature will be labeled, All classrooms will be checked regularly.


Standard #: 22VAC40-185-510-D

Description: Based on a review of medications for 6 children it was determined that one medication did not have an appropriate authorization. Evidence: The medication for Child C was an Epipen and the permission form was for AuviQ. This was cited at the last licensing inspection that was on Jan. 11, 2016.

Action to be Taken: The director will give the parents a new form to be filled out by the doctor but in the meantime will have the parents complete a 10 day authorization form. Ten-day form was completed during the inspection.


Standard #: 22VAC40-185-520-C

Description: Based on review of the three ointments in the Infant Room, it was determined that there was no permission form for one child. Evidence: There was no permission form for the diaper ointment for Child D.

Action to be Taken: The director will assure that parents have filled out the permission form and it will be on file.


Standard #: 22VAC40-185-520-D

Description: Based on a review of over-the-counter skin products and interview with the staff, it was determined that there has been no documentation of use. Evidence: Staff stated that they are not documenting the application of insect repellent.

Action to be Taken: The director will assure that staff and documenting any application of Insect Repellent. She will design a new form.


Standard #: 22VAC40-185-560-F

Description: Based on a review of the menu, it was determined that some meals do not meet the USDA requirements. Evidence: Several days on the monthly snack menu have only one component of the the required two and one lunch needed additional protein.

Action to be Taken: The cook fixed the menu during the inspection.

The Virginia Department of Social Services inspects all licensed child-welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency: “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
