DAY CARE INSPECTIONS: Minnieland Academy of Braemar Cited for Failing to Supervise Children

daycareinspectionslogoThe Virginia Department of Social Services conducted investigations Oct. 7 & 27 in response to a complaint filed against Minnieland Academy of Braemar.

Licensing Inspector Stephanie Reed cited the day care center, located at12700 Correen Hills Drive in Bristow, for sight and supervision and accident reports following two incidents involving a student who was found with a bloody lip.

Virginia Department of Social Services Comments:

A complaint investigation was conducted with the local agency and law enforcement regarding supervision and special care provisions and emergencies on Oct. 7, 2105 and Oct. 27, 2015. Staff interviews were conducted. Documentation, staff and children's records were reviewed, and classroom observation was conducted. The complaint regarding sight and supervision and accident reports was determined to be valid. Please have the plan of correction and the date to be corrected by returned by Nov. 6, 2015 at noon.

Complaint related: Yes

Description: Based on review of documentation and staff interviews, it was determined that not all children under the age of ten years old were within sight and sound supervision at all times. Evidence: Child A, age ten months, was injured and had a bloody lip on Sept. 2, 2015. The parent of Child A noticed the injury at pick up. No staff could explain how the child sustained the injury or when the injury occurred. On Oct. 5, 2015, Child A was again injured and had a bloody lip. Staff B noticed the injury at approximately 4:30 p.m. and notified the parent at pick up. No staff could explain how the child sustained the injury or when the injury occurred.

Action to be Taken: These incidents occurred with a newly mobile infant. The Sept. 2, 2015 incident was not notified or reported to the school until after the mother had been out of the car and came back. The Oct. 5, 2015 incident was reported to the mother verbally, as she was picking up minutes after the lip was seen. An accident was completed that evening and given to the parent the next morning. Description: Based on review of documentation and staff interviews, it was determined that the center failed to maintain a written record of children's minor injuries in which entries are made the day of the occurrence. Evidence: On Oct. 6, 2015, Child A was injured sustaining a bloody lip. Staff notified the parent at pick up, but no written record was provided. Documentation of the incident was recorded the next business day on Oct. 7, 2015.

Action to be Taken: We will retrain staff on more timely documentation. We will ask the parents to wait for the documentation to be finished if the accident has occurred directly before their arrival.

The Virginia Department of Social Services inspects all licensed child-welfare agencies at least twice a year, with at least one unannounced inspection each year.

“In order to determine continued compliance with standards during the effective dates of the license, the department’s licensing representative will make announced and unannounced inspections of the facility or agency during the hours of its operation,” the agency states on their website. “The licensee is responsible for correcting any areas of noncompliance found during renewal or monitoring inspections.”

VDSS cautions that a compliance history is in no way a rating for a facility and online compliance reports do not include consideration that some information regarding “adverse actions” may be the subject of pending appeals.

According to the agency: “Through the administration of the licensing program, the Department of Social Services assumes responsibility to ensure that licensed facilities and agencies provide children and adults with at least a minimum level of care in accordance with regulations prescribed by the State Board of Social Services and Child Day-Care Council.”
